Diclofenac

Diclofenac has potent anti-inflammatory analgesic and antipyretic actions. The mechanism of these actions is inhibition of the enzyme, cyclo-oxygenase in the pathway of prostaglandin(PG) synthesis. PG’s are known to be associated with inflammation. Diclofenac is available as a sodium and potassium salt.The Potassium salt is absorbed rapidly and action sets in much earlier.

Indications & Dosage:

Oral – Inflammatory and De-generative arthritis, RA, OA Ankylosing Spondylitis, acute gout.Low Back pain: 50mg b.i.d. Children over 1year: 1-3mg/kg/day in divided doses.
Dysmenorrhoea: Start with 50-100 mg and increases if required upto 200 mg/day.Sustained release forms: 100mg

IM - Indication as above : 75mg 1-2 times daily.

Children – Not recomemded

Composition:

Diclofenac Sodium B.P. 50mg

Safety Alert:

CL: Active peptic ulcer, hypersens to diclofenac, aspirin or other NSAIDs
SP: Patients with history of GI ulceration, impaired hepatic , cardial or renal function.Pregnancy and Lactation
INT POT-HAZ: Increases blood levels of lithium and digoxin. Inhibits diuretics but potentiates potassium sparing diuretics increases toxicity of methotrexate.
Others – Lowers Serum Concentration of salicylates.
ADR POT-LT: Perforation of gastric ulcers, GIT haemorrhage, blood dyscrasias and anaphylaxis. Others – Peptic Ulcer, Abnormalities of Liver and Kidney Function.
INT-FOOD: Delayed absorption of no clinical significance in long term therapy.

Parafenac (Diclofenac + Paracetamol)

Diclofenac is an NSAID. Paracetamol is an analgesic and antipyretic. When used together the actions of paracetamol set in earlier and provides pain relief before the effects of diclofenac sodium set in.

 

Indications & Dosage:

Oral – Pinful Inflammatory Conditions :
Diclofenac sodium 50mg paracetamol 500 mg(tablet), 1 tablet t.i.d. after meals.

Paracetamol B.P. 500mg

Safety Alert:

CL:Hypersensivity, active peptic ulcer or GI bleeding, history of allergic responses to aspirin or other NSAIDs, acute porphyria.
SP: Analgesic nephrpathy, renal orhepatic impairmrnt, hypertension, cardiac failure, bronchospasm, patients taking durectics.
INT POT-LT: Increased levels of lithium,digoxin and methotrexate
ADR POT-LT: Bleeding or perforation through peptic ulcer, rarely blood dyscrasia or anaphylaxis , acute renal failure – Others – GI disturbances, nausea, vomiting, diarrhea, hepatitis, rash, prolonged bleeding time.
